Zencare Practice Management includes a library of over 85 evidence-based clinical assessments (measures) that you can use to track client progress over time. Measures can be sent directly to clients through the Client Portal and results are automatically scored and recorded in Zencare.
What are Clinical Measures?
Clinical measures are standardized assessments used in mental health practice to evaluate symptoms, measure progress, and support treatment planning. Examples include depression scales (PHQ-9), anxiety scales (GAD-7), trauma assessments, and many more.
How Measures Work in Zencare
- You select and send a measure to a client through the portal
- The client receives a notification and completes the measure online
- Results are automatically scored and saved to the client record
- You can track scores over time to visualize progress

Using Measures with Clients
Once you've identified a measure you'd like to use with a client, here's how to assign and track it.
Assigning a Measure to a Client
Open the client's profile from the Client view and selecting View.
Select Measures, and then select Send Measure either from the center of the screen if no measures have been sent to the client, or from the bottom left menu under Actions.
Next, search for the measure you want to assign and select the appropriate measures. Select the recipient email address at the bottom and finally, select Send Measures.
The client will receive an email notification with a secure link to complete the measure in the Client Portal.
Once the measure is complete, you will receive a notification email.
Tracking Completion
You can see the completion status of sent measures in the client's Measures tab. Measures show as Pending until the client completes them, then update to show the score and completion date.
Viewing Measure Results
When a client completes a measure, the results are automatically scored and saved to their profile. Zencare Practice Management automatically plots measure scores on a graph so you can visualize client progress over time. If you go to a client profile in the Clients view and select View, then the Measures option under Navigate, in the Overview tab, you will see this graph and be able to quickly Compare measures from different date ranges.
You can also view how the client answered the measure question by question by selecting the measure and then selecting View, Download, or Print under the Measures tab.
You can change the date range the measures chart displays under the Compare section by selecting the desired dates and selecting Refresh.
Interpreting Measure Scores
Each clinical measure has its own scoring system and interpretation guidelines. Zencare displays the score alongside interpretive ranges to help you understand what the score means.
Score Ranges
When you view a completed measure, you'll see:
- The total score for that completion
- The score range the client falls into (e.g., minimal, mild, moderate, severe)
- A color-coded indicator to make interpretation quick and easy
Subscale Scores
Some measures include subscales that assess specific dimensions. Where applicable, Zencare will display both the total score and subscale scores.
Clinical Interpretation
Score interpretation should always be done in clinical context. Standardized scores provide useful data but should be considered alongside the full clinical picture, including the therapeutic relationship, presenting concerns, and client history.
Note: Zencare's scoring is based on the original published scoring guidelines for each instrument.
Administering Measures
You can administer measures yourself as opposed to having the client fill out the measures remotely from with ZPM. First, select Administer under the Measures tab.
Next, select the measure you would like to administer.
Finally, go through the measure and select the appropriate options. When you are finished, select Submit.
